What is this portrait effect/how can I get it?

It could be a Gaussian blur in a layer set to about 10% opacity. It could be desaturation along with a bump towards overexposure, even if it's done in post. You can lift the center of the curves line a bit to lighten the image overall, slide the middle levels slider toward the left, or just increase the brightness a bit.
